My 250 bass roll n tuck bass head has 3 jacks on back: Module/tape/output 3to4 ohms.
I figuring this is a 4 ohm head. do I need 2- 15" speakers that are 8 ohms each for impedance of 4 ohms or what?

Yes, two ohm speakers in wired in parallel with be the 4 ohm load that the amp needs to output it's full 125 watts of clean rms power.
If you place a 8 ohm load on the amp it will only output some 75 watts of clean power.
